New Students

Welcome to Masconomet! We're glad to have you as part of our school community and we hope your transition is a smooth one. In order for us to provide the best care for your child as well as meet state requirements, we require the information listed below before your child begins classes at Masconomet.  If your child has any chronic health conditions (i.e. asthma, severe allergies, seizure,  activity restrictions, etc), please notify the school nurse and go to our Chronic Illness page for more information.

• Copy of your child's health records from their previous school
• Completed Immunization Record (click here for immunization requirements)
• Physical Examination within one year of enrollment date
• Emergency Card (for students entering during the school year)
