Registration is open!

(NOTE: The registration will remain open until we reach 250 registered students. After this number is reached, a wait list is created. Once another 30 students register, the waitlisted students will then be able to register.)

 

1. Register through Gerber Tours.

I have attached the instructions from Gerber Tours. The link is on page 3 of the attached Trip Information.

2. After registering for the trip, please fill out the emergency medical form and permission slip. 

  1.    Both are included in the Google Form linked below (both are included in one Google Form) 
    • If you wish to fill out the information on paper, please let me know, and I will send one to you. 

3. If your child needs medication, you will need a doctor's order and fill out the medication administration form

for EACH medication taken on the trip (Prescription AND over-the-counter)

    • The medication administration form is a Google Form, linked below
    • On the form is a place for the doctor's orders to be uploaded
    • EACH medication will be a different Google form/doctor's orders
    • NOTE: Doctor's orders may also be emailed to me, faxed or sent to the school once received from doctor
    • If you wish to fill out the information on paper, please let me know, and I will send one to you

Emergency Medical Form and Permission Slip (Google Form)

EVERYONE must fill out this emergency medical form and grant permission to attend the trip.

Medication Administration Plan

  • ONLY needed if student needs medication on the trip (prescription AND over-the-counter)
  • A separate form is needed for EACH medication
  • EACH mediation needs doctor's orders (uploaded into Google Form, emailed, or brought into school)
