Permission Slip
EVERYONE must have this slip signed.

All students must have a signed permission slip to attend the trip.
Emergency Medical Treatment Form
EVERYONE must fill out this Emergency Medical Treatment form.

NOTE: Other forms are only needed if medication is needed on the trip.
Scholarship Form
Fill out this form and submit it to Dr. McManus in the middle school.
Itinerary (Tentative)
The tentative itinerary for the trip is on the inside of this brochure.

You will receive a final itinerary before we leave.
Medication Order Form (to be filled in by licensed physician)
Fill out ONLY if medication is needed

Use this form for EVERY medication (prescription AND over-the-counter) needed on the field trip.

(NOTE: You may use this form or a form from your doctor's office.)
Medication Administration Plan (signed by PARENT/GUARDIAN)
Fill out ONLY if medication is needed

Along with the doctor's authorization, a parent/guardian must fill out this Administration Plan for EACH medication needed on the trip.
